The style of the character and background isn't so different that it'll be distracting, but the caveat is if you continue to keep the characters and background styles consistent with what you have now. One way to make an ambiguous illness work can be that the patient doesn't want to talk about or acknowledge it. The natural progression becomes that trying to know more about the illness against their wishes makes the character feel violated, and can result in a bad ending.

Unlike Photoshop, I suggest making the artboard the same size as the final resolution of the project. It'll be easier to export quickly, especially if you need to run a script for batch export on multiple files/artboards.

I did a bit of Googling and searching r/gamedev on Reddit and $25 doesn't seem to buy you even one minute :/

It looks like rates start at $50 per minute, but I also know people on Lemmasoft tend to undersell themselves by quite a bit compared to industry standard pay.
