It's displaying like that because the resolution is set as 800x400 on the options.rpy file

Setting the resolution as 800x600 at that file solves that issue (and displays the rest of the screen correctly).

(now, why it only shows the miko outfit?)

I really can't see Narcissu being animated... an actual live-action movie would work better for this plot IMO.
