I write VNs because I adore the storytelling style of visual novels (branching paths! Pictures!) and I want to bring something to the market that isn't yet out there (as far as I can see). Well if most of the game takes place after the weight gain it seems more practical to not show the entire process. If your artist is able and willing to do several sprites it'd certainly be a nice touch, but I'm sure people would understand if it was done in time skips instead. Maybe instead of one...

IMO it very much depends on the character and the exact mood they're expressing. Like if they're scared, it should be a full body thing, even if it's just subtle stuff like stiff shoulders and wide eyes. If they're just pleased, then a subtle smile would work. People might not glance up at the sprit...

I have to warn you that many writers consider switching POV's a very bad move. I know that it's risky since it's easy to mess up with and overdo or just be bad. Still though, it's either switch to another character or switch to third person in order to get some important events in. If switching POV...

Switch PoV's after a chapter and feature the name of the narrator somewhere under the chapter name, or something.
