being a vocaloid fan, i recognize all these pictures. i don't really know what to say, just, um.
vocaloid themselves don't limit any derivative work but i dunno for the artist of the original vn you seemed to be ripping off.

It won't be confusing if you're putting on a first-person viewpoint (which is the standard of most visual novels), since basically mc=narrator.

However, if you're doing a third-person viewpoint, it might get confusing. It's better to just do it like a dialog and, for example, put it in a bracket.

For me, working with less is better since they are... Say, more memorable. I often get lost in track with vns containing a buckload of characters, like for example naruto/ fairy tail/ just name any recent shonen manga here :/

You can define hotspots as animations, since basically ren'py takes any displayables, not just images, for a hotspot.
