This probably won't be the best advice since I'm also pretty easily addicted to - anything D:
But here goes:
1) Try to get more addicted to shorter games lol
2) Try to set up a reward system - "After I do this homework/ set of problems, I'll play 15 minutes of the game!"
3) Get your parents to order you not to play the game (my last resort >3>)

I tell my parents to keep the PC away from me. I have internet addiction, and it's hard to keep me away from it.
So, I lock the PC door and let them keep the keys.

Pulling out of a game takes way too much willpower... Solution: don't buy/download any new games. Sort of makes it easier.

I did that when I patched TMGS1 and TMGS2 and I played it 24/7. Everyday I would get into bed at 10pm and wouldn't actually sleep until 5am on a school day as well. I stopped when my mum took my DS off me (T_T) but I guess it helped.
Also, I find that another way to stop an addiction is playing it forever and ever. For example, I had a phase where I had to just read manga all the time and I never got any homework done, etc. I gouged myself with bishies and typical shoujo stories until I actually got a bit pissed off with it. Then, I kinda of naturally got used to not reading manga.

They always say you hit rock bottom before you get help. Gorging on something usually will make you hate it. Too much of anything is bad for you. This usually happens to me a lot, I tell a family member that I like chocolate and then they buy me 2,000 chocolate bars for my birthday and then I just hate chocolate. THANKS.

I'm not going to recommend more of the addiction, but maybe you're addicted because you're not playing it enough? (not true in most cases, but some) Distance makes the heart grow fonder, etc. Live with someone, and you'll hate them. See and do everything there is to do in a game and ... meh.

Realize when it stops being fun and becomes a chore. As soon as it becomes a chore, find something that is actually fun to replace it and maybe your body will realize what an awful chain you're in.

As far as schoolwork is concerned: find someone in your class and schedule a "pizza-and-study session" where you can get away from the computer and get some studying done and have some fun at the same time.
