It's just something that's been on my mind.
The worlds idea is fine. You might want to make sure that this is okay with anybody close to you struggling with mental disorders. This could come across as not caring about them which could cause anger or suicidal tendencies. In a general sense, an interactive entertainment confronting BPD should be helpful towards eliminating the stigma.
I'm not sure how much you have researched Borderline Personality Disorder, but the name doesn't describe it properly. There are a few stigmas and ideas I can write down that might help you with your project. The only thing I ask in return is the pleasure of playing it when you are complete.
Dissociate which can be described as zoning out. This can be the mind blocking out a painful event. A common example would be sexual abuse. (Dissociate World where you brought up a painful topic.) This world could be blank and even be your first world because this commonly involves a detachment from reality. Remember that these problems can be worked through.
Impulsiveness, spending sprees, alcohol abuse, eating disorders, sex, and running away. When it comes to writing impulsiveness has a lot of material to work with. I will separate these.
1. Eating disorders commonly have a root with this disorder. Neglect from a caregiver, which might cause later problems and have them manipulate others to feed them so they feel important enough to live. This is very problematic, I got over this by my boyfriend teaching me how to cook in a private environment. Even though I already knew how to cook. So you could spend quality time with your sister with nobody else in the dream. Teaching her how to cook.
2. Spending sprees this is hard since sisters kind of go shopping to bond. Just keep the purchases at a minimum.
3. Sex sometimes becomes uncontrollable, which to the person with BPD can be fun. Lovers though can experience distress and can set (difficult, not impossible)rules to seem affectionate, but overwhelmed.
4. Running away, people with BPD often feel shame and guilt after running away.
Self-harm, "18% of men diagnosed with BPD commit suicide" luckily girls are down to 8%. Maybe you should stay away from this subject. Groundbreaking if you do it right. Just matters what kind of a writer you want to be.
Intimacy, people with BPD strongly desire intimacy or even if I dare say have stronger feelings of love. This world could be a craving for somebody to connect with and finally connecting with her loving sister. So cute, I hope you use this idea, it makes me wish I had a sister like you growing up to help me.
