^Every time I see someone 'accidentally fall and kiss' someone else, I expect one or both to go, "ARGH OH MY GOD MY TEETH!", but they never do. :C

I don't think I've seen many accidental fall-and-kiss, just fall on and be embarassed about the chest contact. I may not be reading enough of them.
(Of course, in the case that comes most clearly to mind, she turned out to be his sister anyway, which somewhat limited the sexual tension.)

I wouldn't mind a subversion of the "oops" situation-- if the insinuator was planning to do something, try to make it LOOK like an accident, and ended up getting into an even more awkward situation.
1) accidentally knocking the object of affections over, they hit their head and are lying on the floor in a comatose state (but this could also be a scenario for a really bad h-game...)
2) alternatively, accidentally knocking out the gorgeous object of affection's tooth. or two, or three. their million dollar smile is destroyed. you are forced to pay off their dentist bills and on top of that, they're really mad at you. (and this almost sounds like the premise of a weird but possibly entertaining game)
3) if there are any possible results more terrible than #2 let me know
it's unrealistic and cliche, sure, but if it's played for laughs I don't really see it as such a big problem.

You accidentally push the object of affection in the direction of the stake ... which leads to him/her falling on it, it piercing his/her body ...
