I think you can certainly have a horror VN without the monsters. Really, in order to be frightening, all a VN needs to have is something that can hurt you or those you care about.

Do ancient curse or some pyscho serial killer count as monsters?

Dark Water tells a story about a cursed water that makes people hallucinates (and a very terrible one that could really kill you). No monster in it but it still a horror.

Humans and monsters are different, but have the same brutality.
Hmm... Maybe some psychologist horror, like someone who is afraid to everyone near him, feeling shock, or has a phobia towards something...

Divi-Dead is still one of my all time favorite games. There were no 'monsters', per-se, but several of the characters were... not entirely human. They LOOKED human, but they had abilities above and beyond.

Yeah, the translation was crap, but it was actually a pretty good game.
