I personally think this problem is less of an issue in visual novels, since you can use many visual cues to show who is talking. In books and scripts, that is more important because you don't have anything beside the name to show who is talking.

Even just using a different color for a different talking character helps a lot in avoiding confusion.

And by the way... After reading One hundred years of solitude, I realized the problem was bogus for the most part. You can have a lot of characters named all the same and all be easily distinguishable just from what they do and how they talk. I think in the end that is the most important thing when writing.
