How To Credit Me:
Wherever the credits are just put my name there. So if there's credits on a website, put it there. If they're at the end of the VN, put it there. I'd prefer to be credited with my real name, Jake Bowkett.

Also do you have any specific terms (e.g. credit each time music is used, a link to your website, credit each song that is used individually or just your name under music at the end credits is fine)

If you want to do per track credit that's fine, but a general credit is also fine -- up to you. I'd prefer to be credited with my real name, Jake Bowkett.
