Let's see.
I absolutely love SHMUPs (shoot'em ups) so Cave games are high on my list but surprisingly the hardest one to me is pretty old : Hellfire
It was developed by Toaplan, the guys who did Zero Wing (remember "All your base are belong to us" ? ^^)
God did that game have a fitting name...

Actraiser II for the SNES
It's not only a game where you descend to hell to fight Satan... it's hard as hell.
I have been trying to beat this thing ever since it came out in the early 90's.
Recently I played it again on an Emulator so I had tons of tools you don't have on an SNES.
There are like dozen cheat codes for this + some hacks, trainers, editors etc..
I used them ALL + every cheat and trick I could think of + additional hacks, trainers and editors + time slow down and emulator save function etc..
Still I never made it past the first 3 levels (out of TEN) and in EASY MODE!
This is definitely the hardest game I ever played and I am a life long gamer. I wouldn't be surprised if it is THE hardest game. It's a shame it doesn't receive more recognition for that. Besides it's a pretty damn good game so the motivation is high - but it's really impossible... damn it's so frustrating

I've played it and I don't think it's so hard that it's unbeatable especially since you're using cheat codes. Anyway, save + load state tricks would be your best friend no matter what kind of game you play.

This honor would have to go to Dark Souls, which is also on my "best games of all time" list. It's a steep learning curve, but not THAT hard once you learn all the tricks, and 100% worth it.
What makes it great isn't just that it's challenging, but it's also very atmospheric and immersive. QWOP, that game is hilarious though. Not all that addictive but certainly really difficult.

I fricking hate it. I love to collect lava to power my stove, but I die in 80% of all times that I go underground.
It's such a annoying, horrible and stupid game (seriously, they could at least make it that your stuff doesn't fall IN the lava and gets destroyed, but floats around next to the lava so you have a chance to get it back). But whenever I'm terribly bored, I play it. Making my castle bigger, breeding the 30th sheep, making the pigs nearby go extinct and...going underground (to die)...
It's so illogical, funny and strange, you can't stop playing it xD
