Allow players to add comments to their save data? (like F/S)

Like the title says, how do I allow the players to add a comment to their save data like in Fate/Stay Night?
Also, say for example that every scene in my game has a corresponding title, how do I add that to the information on the save data?

Also, the way my save/load screens are, the screenshots are a bit too big for the erm.. frame(?)/thumbnail(?). How do I adjust the size of the screenshot?

There isn't a way to add comments to the save data. While I know how to actually do the work, there are a lot of other problems - like what happens if the text is too large for the area alotted.

To change the screenshot size, modify config.thumbnail_width and config.thumbnail_height.
