Eyes of a Dead Fish

Was the ending supposed to be a twist? It was a bit confusing because at first it looked like

the narrator's friend was the one who actually died in the car accident, since the two at the end look like the two kids from the past, but looking back at the picture with the kids the kid without the glasses seemed to be the narrator and he's dead at the end. So was it actually the narrator who died?
